The upper cervical spine — particularly C1 (atlas) and C2 (axis) — contains proprioceptive receptors that feed positional information directly to the brain's balance centres. When these vertebrae are misaligned, the signals they send become distorted, creating dizziness, unsteadiness, and a spinning sensation.
Cervicogenic dizziness is frequently overlooked because it mimics inner ear conditions like BPPV. The distinguishing feature is that cervicogenic vertigo is often accompanied by neck pain or stiffness, and symptoms change with head and neck position.
